Extended Aged Care in the Home (EACH) Packages is a care option that provides an alternative to high level residential care for frail aged or older people with disabilities in the community.
The EACH Package can make the difference between remaining at home or moving into a residential care facility. Foe example, assistance with a range of the following:-
- Personal Care
- Meal Preparation
- Emotional Support
- Cleaning and Laundry
- Shopping, Transport and Recreation
- Medication Assistance
- Nursing
- A key aspect to the success of an EACH package is the high level of support received from Care Coordinators who provide coordination of all services plus, where the needs are quite complex, a range of case management options
A person assessed as needing high level care will be eligible to receive an EACH package. GP’s usually make the referral to the ACAT team or Commonwealth Carelink centers can be contacted on 1800 053 222 for local ACAT information. A contribution towards the cost of the service is usually required and is determined on an individual needs basis, according to Government guidelines.
