Community Aged Care Packages (CACP)

'Community Aged Care Packages' are a care option that provides an alternative to low-level residential care for frail aged or older people with disabilities.

Community Aged Care Packages are provided across Metropolitan Perth and South West Regional W.A., operating as "Aged Care Services Australia Pty Ltd". Including packages designed specifically for people with dementia.  Community Aged Care Packages are also provided in Alice Springs, N.T., operating as "Community Care Services Northern Territory Pty Ltd".

 Arrangements with each recipient of the care packages are strictly confidential.

ACSA embraces the Wellness Approach and encourages all service recipients to remain as independent as possible by building on skills and abilities so as to prevent premature admission to an aged care facility.

A range of the following services can be negotiated to enable individuals to remain in their own home: -

  • Personal Care
  • Meal Preparation
  • Social Support
  • Cleaning and Laundry
  • Shopping, Transport
  • Medication Assistance

Anyone can request a community care package. However, prospective clients must first be assessed by an Aged Care Assessment Team their own home.  GP's usually make the referral to the ACAT team or Commonwealth Carelink centres can be contacted on 1800 053 222 for local ACAT information.

This service is funded by the Commonwealth Department of Health and Aged Care. A contribution towards the cost of the service is usually required and is determined on an individual basis, according to Government guidelines.
