Home and Community Care

The Home and Community Care Program is a joint initiative of the State and Commonwealth Governments.

The aim of the program is to provide low level services to enable the frail aged and younger people with a disability to remain living independently in their own home.

Funding is provided to assist with:

  • Personal Care
  • Domestic Assistance
  • Social Support
  • Respite

Access to the service is by self referral to our Package Operation Manager who will conduct an eligibility assessment. Once eligibility has been determined a Care Coordinator will discuss your needs and complete a care plan in consultation with you. Aged Care Services Australia is also funded for HACC service to people from non english speaking backgrounds.

HACC Dementia Specific

In addition to our basic Home and Community Care services is a program designed specifically to meet the needs of people with the early onset of dementia and their carers. The program focuses on providing services which assist consumers to document their memories by developing Living Histories at the same time as providing basic services.

A contribution towards the cost of the service is usually required and is determined on an individual basis, according to government guidelines.
