| TSC Coordinator - Community Care |

We are offering an exciting career opportunity for a Community based Program Coordinator to join our Transition Care Service (TCS) team in Bunbury coordinating and case managing services which assist in the rehabilitation of older people following an acute episode of hospital care.
Reporting directly to the Unit Manager, this role will hold a variety of responsibilities including;
- Undertake appropriate assessment, care planning, goal setting, monitoring and review of clients’ support needs using the Wellness Approach.
- Discharge planning
- Developing and maintaining networks. Liaising with State Department representatives, local Hospitals and GP’s, and the Community Division in Perth
- Monitor client and program budget
- Provide relief cover to Coordinator in Busselton
- Provide budget and statistical reports to management
- Ensure care delivery standards are maintained
To be successful in this position you will have;
- Aged Care or health related experience, preferably with a social work, physiotherapy or nursing background
- Tertiary qualifications in an allied health profession or health related discipline
- Knowledge of Transition Care Therapy Program, Community Rehabilitation or similar
- Previous experience in program coordination and budget management
- Eligibility for full membership of a professional body
- High level computer literacy
- Evidence of highly developed interpersonal skills
- Ability to work in a team environment and develop a team approach with all staff
- National Police Clearance

We currently require an experienced Registered Nurse, on a part time basis - Tuesday & Wednesday, day shift only.
Reporting to the Facility Manager, you will be:
· Responsible for the holistic nursing care, consultation & intervention for residents
· Providing a ‘Hands on’ role as an experienced aged care nurse
· Completing ACFI assessments
We are looking for someone who has:
· Current WA Nurses’ Board registration
· Aged care clinical experience
· Strong communication skills
· ACFI assessment experience

Reporting to the Care Coordinator, and working closely with the Registered Nurse, the role is responsible for
- Providing direct care to residents under the direction of the Registered Nurse/Care Manager
- Assisting in the development of care plans for residents
- Implementing the care plans in consultation with residents, families and other members of the care team
- Administering medications in line with WA Nursing & Midwifery Board standards, and St Ives policies and procedures
We are looking for someone who has:
- Enrolled Nurse (or equivalent) qualification
- Current WA Nursing & Midwifery Board Registration
- Clinical care experience
- Sound working knowledge of the Aged Care Standards & medication administration standards

No Qualifications? Traineeships Available!
St Ives is a Registered Training Organisation offering high quality, nationally recognised training for candidates in Aged Care. Our training program has been running since 2008 and is specifically designed to meet the needs of the aged care industry.
We currently offer Government funded traineeships in Certificate III in Community Services (Aged Care) to eligible candidates. The course involves both classroom activities and paid on-the-job work experience in the community. Students on Government funded traineeships will be paid to learn, and gain valuable skills and experience whilst working in the community.
We are currently running Certificate III in Community Services (Aged Care) course both north and south of the river. If you have partially completed your studies, contact our training department to see if you can gain recognised prior learning for your studies to date.
